Before you get too offended, consider the colloquialism "talking shit," which basically means saying a lot of unnecessary things that are negative about someone (often behind their backs). Then you will realize that the shithole metaphor has more than one layers here, and that our current president does this a whole lot and that the attention it receives takes away from the attention people pay to the policies that are being enacted under his administration.
